The Entry and Insurance Forms have changed for the
2010 season. Please use only forms published below.

NESS Entry Form	One form for each paddler, good at all NESS Races. - (ACA Waiver* is also required)
ACA Waiver for Adults*	This Form is to be filled out and signed by each adult paddler.
ACA Waiver for Minors *	This Form is to be filled out and signed by each paddler under eighteen years old and a parent or guardian..

* All participants must sign Insurance Waivers. ACA members must also affix a valid membership numbers. All others, including USACK members, must pay the $5.00 Event Membership Insurance Fee. Race Organizers will be billed $5 for each invalid ACA number, as determined by ACA HQ Staff. Click here to check the status of your ACA membership.
